Are You Taking the Right Probiotic for Your Gut?
You walk into a chemist or health food shop looking for something to help your bloating, constipation or generally unsettled digestion, and you are faced with an entire wall of probiotics.
One contains 10 billion bacteria. Another contains 50 billion. One has three strains, another has fifteen, and almost every bottle promises to support digestion, immunity and a healthy gut microbiome.
It would be completely understandable to assume that the product with the highest number and the longest list of bacteria must be the best one.
Unfortunately, probiotics are not quite that simple.
I use probiotics regularly in practice, and I think they can be extremely helpful when they are chosen for a clear reason. However, I also see many people who have spent months taking random probiotics without noticing any improvement. Some feel no different at all, while others find that their bloating, wind or digestive discomfort actually becomes worse.
This does not necessarily mean probiotics are unsuitable for them. It may simply mean they are taking the wrong probiotic for what is happening in their gut.
More strains do not automatically mean a better probiotic
One of the biggest misconceptions around probiotics is that more must be better.
More billions. More species. More strains.
However, a product containing fifteen strains is not automatically more effective than one containing only two or three carefully selected strains. What matters is which specific strains are included, how much of each strain is provided and whether those strains have evidence behind them for the reason they are being used.
Different probiotic strains can behave very differently within the body. Even strains belonging to the same bacterial species are not necessarily interchangeable.
One strain may have been researched for antibiotic-associated diarrhoea, while another may have been studied for constipation, immune health or symptoms associated with irritable bowel syndrome. Seeing a familiar bacterial name on the label does not necessarily mean the product will have the same effect as another probiotic containing a different strain of that species.
This is why I rarely choose a probiotic simply because it contains the highest number of bacteria or the greatest variety of strains.
The combination needs to make sense for the person taking it.
Will the probiotic survive long enough to be useful?
The quality of a probiotic also matters because the organisms need to survive quite a difficult journey before they can have an effect.
Your stomach is naturally acidic. This is an essential part of digestion and helps protect you from potentially harmful organisms, but it also creates a challenging environment for probiotic bacteria.
After leaving the stomach, the organisms are exposed to bile and digestive secretions as they move through the gastrointestinal tract. Some probiotic strains are naturally more resistant to these conditions than others, and the way a product is manufactured and stored can also affect how many organisms remain alive.
Some brands use delayed-release capsules or protective coatings to help the bacteria survive the stomach. Storage conditions, moisture, heat and the length of time a product has been sitting on a shelf can also influence its quality.
This does not mean you need the most expensive probiotic available, but it does mean the number printed on the front of the bottle does not tell you everything.
A product may advertise 50 billion organisms, but that number is not especially meaningful if the strains have not been clearly identified, the bacteria have poor stability or the potency is only guaranteed at the time of manufacture rather than until the expiry date.
When I am choosing a probiotic, I am looking at the reputation of the manufacturer, the specific strains included, the way the product is stored and whether there is evidence behind the formula for the reason I am recommending it.
What do you already have in your gut?
This is the part that is often overlooked.
When you take a probiotic, you are not adding bacteria to an empty digestive system. You are adding them to an already established ecosystem containing hundreds of different microbial species.
That existing ecosystem is different for every person.
Two people can take exactly the same probiotic and have completely different experiences. One person may feel noticeably better, while another notices no change or feels more bloated.
Your existing microbiome, diet, medications, bowel habits, digestive function and immune system can all influence how you respond.
Many general multi-strain probiotics contain several Lactobacillus species. These are beneficial organisms, and there are many situations where Lactobacillus strains can be very useful.
However, it does not follow that every person with gut symptoms needs a large dose of multiple Lactobacillus strains.
Someone may already have a high representation of lactate-producing bacteria in their gut, while having very low levels of other important organisms such as Bifidobacterium or bacteria involved in producing butyrate.
Butyrate is a short-chain fatty acid that helps nourish the cells lining the colon and supports a healthy intestinal environment. Bifidobacterium species are also important for maintaining gut balance, supporting the immune system and helping break down certain fibres.
In this situation, repeatedly adding more Lactobacillus may not address the areas that actually need support.
It does mean that taking a broad probiotic without understanding what is already happening can sometimes be little more than guesswork.
When a general multi-strain probiotic can be helpful
There are certainly situations where I am comfortable recommending a high-quality, reputable multi-strain probiotic without completing microbiome testing first.
A common example is around a course of antibiotics in someone who is otherwise healthy and does not have a long or complicated history of gut symptoms.
Antibiotics can disrupt the intestinal microbiome, and certain probiotic strains have been shown to reduce the likelihood of antibiotic-associated diarrhoea.
Even then, I prefer to use a product that contains clearly identified strains and has evidence behind it for that purpose, rather than simply choosing whichever product has the most bacteria.
I also explain that a probiotic is only one part of supporting the microbiome after antibiotics. It does not necessarily restore the gut to exactly how it was beforehand.
Recovery may also involve eating a broad range of plant foods, gradually increasing appropriate fibres and prebiotic foods, supporting regular bowel movements and giving the microbiome time to recover.
When testing may be more useful than another random probiotic
When someone has had persistent bloating, constipation, diarrhoea, abdominal pain, food reactions or digestive symptoms that have continued despite trying different diets and supplements, I often want to understand more before recommending another probiotic.
Comprehensive microbiome testing can provide information about the bacterial species present in the gut, microbial diversity and the balance between different groups of organisms. Depending on the test, it may also provide information about inflammation, digestive function and the wider intestinal environment.
Testing help us move away from making assumptions.
It may show that someone has very low Bifidobacterium, low microbial diversity or poor levels of important butyrate-producing species. It may also show that they already have a significant amount of the types of bacteria being added through their current probiotic.
This gives us a much clearer starting point and a much more effective outcome.
Sometimes a targeted probiotic is appropriate. At other times, the more important first step may be increasing the right types of fibre, improving food diversity, addressing constipation, supporting digestive function or investigating why the microbiome became disrupted in the first place.
For someone whose gut has not been well since food poisoning, repeated antibiotics, a significant illness, prolonged stress or another clear turning point, simply adding a generic probiotic may not be enough.
You may not need the strongest probiotic
Probiotics can be incredibly useful, but they are not all the same and they are not one-size-fits-all.
The probiotic with the most strains is not necessarily the best one. The highest number of bacteria does not guarantee a better result, and the product that worked beautifully for a friend may do very little for you.
The right probiotic depends on the specific strains, the quality of the formula, the reason it is being taken and what is already happening within your gut.
When you have ongoing digestive symptoms, it may be worth asking whether you really need a stronger probiotic, or whether you need a clearer understanding of your gut in the first place.
Sometimes the answer is not to add more bacteria.
It is to stop guessing.
